Transportation Services
POLICIES AND PROCEDURES
Scope of Services:
We offer HIV positive individuals round-trip transportation, by appointment
only for:
- SWC programs
- PATF Food Pantry
- Wellness and health related programs offered by other local AIDS
service organizations
Limitations of Services:
We
are not able to provide transportation for moving personal property,
shopping trips, personal errands, ambulance service, or individuals
who are not HIV+.
Van territory is limited to our service area as described in our brochure.
Transportation is scheduled on a first-come, first-served basis and
is scheduled as available.
Transportation will not be scheduled when weather conditions are hazardous.
Wheelchair passengers must provide a wheelchair and arrange for persons
to be present to assist them into and out of the van. Van drivers are
not permitted to lift or carry members.
Members traveling with children who require baby restraint must provide
a baby seat and install it when boarding the van.
Guests are not permitted to ride with members. However, caregivers are
permitted to accompany members.
Scheduling Procedures:
Members must request service at least 24 hours in advance of appointment
by calling the Shepherd Wellness office. We will call members to confirm
our transportation schedule.
First time members must complete an "Member
Information Form" (137 KB) when scheduling service or when
boarding the van. (In order to download this form, you must use the

Passenger Policies:
There is no charge for transportation service and drivers are not permitted
to accept tips.
All passengers and the van driver must use seat belts.
No eating, drinking, or use of tobacco, illegal drugs, or alcohol is
permitted in the van.
No stops are permitted between scheduled destinations.
Pets are not permitted in the van.
Van drivers are not permitted to lift or carry patients, do personal
errands, stop between scheduled destinations, or make telephone calls
for members.
If members do not show up for a scheduled pickup, they may not
be scheduled for future service.
Foul language or disruptive behavior is not permitted on the van.
Members who violate this policy will not be scheduled for future
service.
