Become a Member of Shepherd Wellness Community

If you are HIV positive, it’s easy to become a member of the Shepherd Wellness Community. Just fill out and return our Member Information form and certify for Ryan White eligibility.

Once you are a member you will receive:

• Membership in our caring supportive community
• Our bi-monthly Words of Wellness newsletter mailed to you
• Regular phone calls reminding you of upcoming events at SWC
• Free access to our programs and outings (when both up-to-date forms are on file)
